Polycystic ovarian syndrome (PCOS) is a disorder of hormone and very much prevalent among women of reproductive age. Women with PCOS may have different or prolonged menstrual periods or excessive levels of male hormones (androgens). The ovaries can develop numerous small collections of fluid (follicles) and cannot release ovules regularly.

  • Menstruation cycle:- Although some women have regular periods with PCOS, androgens of high level and also the hormone insulin can interrupt the monthly cycle of ovulation (when the eggs are released) and menstruation. If a woman has PCOS, then her periods may be ‘irregular’ or stop altogether. In some girls, PCOS is a cause of periods not starting. The average menstrual cycle is 28 days with anovulation, but in between 21 and 35 days is considered “normal”. An ‘irregular’ period cycle defined as:
  • Eight or fewer menstrual cycles per year.
  • Menstrual cycles are more significant than 35 days.

As menstrual cycles lengthen, ovulation can stop completely or only occasionally. Some women with PCOS also experience more intense or milder bleeding during their period.

  • Excess hair (hirsutism):- Hirsutism is the excess of hair on the face and body because of the high levels of androgens that stimulate the hair follicles. This surplus of hair is thicker and darker. Hair usually grows in areas where it is more common for men to do so, such as the sideburns, chin, upper lip, around the nipples, lower abdomen, chest and thighs. Near about 60% of women with PCOS have hirsutism Women with PCOS of ethnic groups with a tendency to have darker body hair (for example, the populations of Sri Lanka, India and the Mediterranean) are often more affected by hirsutism.
  • Hair loss (alopecia):-For various women with PCOS, the high level of androgens causes the damage or thinning of the hair on the scalp in a “male” pattern: a line of frontal hair in retreat and thinning on the scalp.
  • Acne:-If a woman has PCOS, the higher level of androgens can raise the size of the oil-producing glands in the skin, which can cause an increase in acne. Acne is common in adolescence, but all young women with PCOS also are liable to have more severe acne.
  • Reduced fertility:-High androgen levels and high insulin levels can affect the menstrual cycle and prevent ovulation (the release of a mature ovum from the ovary). Ovulation can bring to an end altogether, or it can happen irregularly. It can make it harder for women with PCOS to conceive naturally, and some women may also have an increased risk of miscarriage. However, this does not mean that all women with PCOS are sterile. Many women with PCOS have children without any need for medical treatment for infertility. Others may require medical support. But in general, women with PCOS have an equal number of children as women without PCOS (research by the researchers).

As overweight can increase fertility problems, it is essential to exercise frequently to maintain a healthy weight and prevent weight gain. For those women who are overweight, even 5-10% weight loss will improve fertility. In addition to all this, late diagnosis of PCOS and problems with weight control can make a woman feel discouraged and helpless.
